If you're willing to install Apache in addition to IIS, it's quite straightforward.
You can use a pre-packaged Windows installer like XAMPP to install Apache, the mySQL Database and PHP.
After that, you install Mantis as usual.
Don't know about IIS, though. In theory, it should propably run fine ...

Database management all messed up
Mantis keeps bugging me about updating my database structure.
However, every one of the supposedly required updates has been implemented already. To satisfy Mantis, I would have to drop columns, then have the update function create them again, and so on.
