I also met a similar trouble and the trouble is caused by $g_email_send_using_cronjob.
When the settings is ON, all notification mail are queued into mantis_email_table and the mail are not sent out.
I think the setting is really beneficial because I feel dissatisfied the performance in response ...

I'm running mantis 1.0.5 with mysql that is ran in another server.
But I have not been able to run it yet by the error which is written in subject of this topic.
The database server and the mantis server are running at same network segment.
