If its empty your upgrade did not work at all.
In that case you need to start again

When upgrading to 1.2.21 (or 1.3), did you follow the instructions in the admin guide?

Suggest to create a plugin that kicks in after an update. There you can do validation and initiate actions.

How old was the backup, from the same mantis version?
Any change in PHP or mysql?

No it will not for now. I will change my setup to display all messages and will fix them somewhere this week. Then I will post version 3

Change error reporting to Error only, it will remove the system notices. That way you can continue. I will review these notices later

What I see is that you are not using the templates I provided so something is not working on the Mantis side.

This output seems to indicate that you did not make all changes I put in place since this is not HTML enabled output.
Did you use the provided email_api.php and did you make the correct changes in core/config_inc.php?
