Add Note refactoring

Hello everyone,

Having had a chance to read though most of the source I can see there is an amount of refactoring which is needed for Add Bug Note. In adding a Time Tracker I had to change the same form on about ten pages. Really this should be factored out and included in as an external file. If you split the functions up into seperate pieces then you can have a simple / advanced view at no extra cost.

Plus you only make changes to one place.
RoadMap: Get rid of redundant code by providing one view page rather than simple/advanced/print).
