Add descriptions besides custom field created or helplink

Hello,
I would like to add description besides a custom field to help user to fill out well the custom field value.
I have found Something on this link : http://deboutv.free.fr/mantis/
But there is no details.
If someone have do the same things before, I will appriciate his help.
