Sending e-mail to an address stored in custom field

I'm not a PHP scripter so need some help here.


I have a custom field (field_id = 21) that stores an email address per issue.

I would like an email to be sent to the address when notes are changed (detailing the notes entered) or when the status is changed.

Any help in the right direction appreciated.
