I had installed MantisBT1.2.5 along with Mysql & Apache
Installation done successfully in production..

INSTALLATION DONE ON WINDOWS

But whenever we raise any issue/ update any issue system goes drastically slow and throws the following error

APPLICATION ERROR #2800
Invalid form security token. Did you submit the form twice by accident?

Changed the security settings as well but didn't work out

$g_form_security_validation = OFF

so again reverted to default settings

Did you check what user heatherk wrote?
