Error in creating new users/resetting password

Im running mantis 1.0.0rc1 on php 5.5.9 ubuntu14.
I am not able to create new users, the same time when trying to reset password I get error:
APPLICATION WARNING #100:
Configuration option 'from_name' not found
Im seeing below error in the apache2 logs
