Email works, but no email notifications

Email works. New users get signup emails. But they don't get any email notifications on new bugs or status changes. As far as I know I have all the right boxes check but nothing. This appears in the .log file

Which I guess means some notification setting is off, but I can't figure out why?

I have all those set:

I still get that in the logs and no email is sent. I must be missing something.

Check email settings on page "My Account" > "Preferences"
