M bug_monitor_list_view_inc.php

• Fixed a problem where the names in the monitor list were coming up empty since it seems that the new implementation of db_fetch_array() does not return the fields indexed by index/column_names anymore, it only returns the fields indexed by column names. I am not sure if this will be causing problems elsewhere.
