Fixed 0003210: bug_view_advanced_page.php (and simple) doesn't show the project name.

• Show project name before category (similar to View Bugs page when "All Projects" is selected, eg: [project] category)

• Dropped support for $g_show_project_in_title since it doesn't make sense anymore. It made sense to show the
  project name in the top when it was always consistent with what the user is viewing, but now it is misleading.

M core/html_api.php

• Don't include currently selected project name (which may not reflect the current bug) in the title or the header.
