Upload is not working

Iam using Mantis 1.0.5 version.

While iam uploading document in my bug_report_Page.PHP. The document is not uploading.

IAm Using MYSQL as database

Iam getting an error message that

"APPLICATION ERROR #15
File upload failed. File is not readable by Mantis. Please check the project settings

Please use the "Back" button in your web browser to return to the previous page. There you can correct whatever problems were identified in this error or select another action. You can also click an option from the menu bar to go directly to a new section."

Please help me in this

Thanks for ur information. Its creating tmp file with empty. The temp file contains no data.

I have given the write permissions. Upload is working fine. But iam unable to find the location where these are files are storing.

Default store in database.
Did you config to store in Disk?
