One user cannot report issue

I have one user who can not report a new issue (create a bug report). When he first noticed this he also noticed that he was now a "manager"--though we don't understand why that would prevent him from opening a new bug. Then I put him back as an "administrator" but still he can't report and issue. When he tries to submit the issue nothing happens; i.e. click "submit issue" and the page does nothing--almost like the button doesn't work for him.

I don't even know where to look at this point. Can somebody point me to where I should look to figure this issue out?

FWIW, I've recently (December) upgraded from 1.0.6 to 1.1.4->1.1.5->1.1.6.

Which browser is he using? let him try a newer/different one.
