Generating a report displaying issues with files attached

I need to generate a report of some kind that will show me the issues with files uploaded to them, including the file name. I'm not very farmiliar with mysql, but I'm assuming running queries against one of the tables in the database is my best bet.

My main questions are:

Is there an easier way to do this?
and/or
Where can I find which tables in the database hold the information I seek?
