Error when reseting password or confirming account

I get this error everytime someone tries to confirm an account or reset a password.

 SYSTEM WARNING: session_destroy() [function.session-destroy]: Trying to destroy uninitialized session

SYSTEM WARNING: Cannot modify header information - headers already sent by (output started at /home/picard15/public_html/bt/core/error_api.php:166)

Then if you go to the next page you get this error:

APPLICATION ERROR #2800
Invalid form security token. Did you submit the form twice by accident?

Anyway to fix this? Or is there a version of mantis that has fixed this?
