I also have this issue. This appears to be the same as issue 9999. The 'fix' there is simply to turn off form validation.

Set up:
Mantis 1.1.6
OS: Suse linux
SQL: MySQL 5.0.26
PHP: 5.2.5

Client:
Opera 9.25
on Win XP.

I put log_event calls in bug_report_page.php and in form_api.php. When I click on 'Report Issue', the form loads, but no logging occurs (in Opera). This indicates that the browser is not reloading the page.

When I manually force the page to reload (either 'reload' button or F5), then I get the expected logging output.

I don't get this error using either Firefox 2.0 or MS IE 7.

I'm marking this resolved, under the assumption that you are using a proxy server between your browser and the server, which is causing the problem with incorrect page caching. In this case, issue 0009999 "solves" this by allowing you to disable form security validation, at the expense of potential security risks.

If you do not have a proxy server in your set up, feel free to reopen this issue and attach more information about your network setup, your server application/versions, the browser/browsers you are using, etc.
