If I have three Enum fields say for first name, middle name, and last name and I wish to have them all depend on each other, can this be accomplished with this plugin?

I.E say we have:
John Jack Doe
John Jack Smith
John Jill Zeebra
Mike Frank Tester

If I were to pick John as the first name, I'd want middle name drop down to only contain available middle name Jack and last name to contain Doe, Smith, and Zebra. If I then picked Jack, I'd only want last name to contain Doe and Smith. It's more or less multiple dependencies. I'm trying to figure out how to keep track of what customer called in the ticket and don't want to store First Name, Middle Name, Last Name, Phone Number, Office Number in one field.
