The version field appears twice in the update simple form page while the fixed_in_version field is missing. I guess it was a mistake.

I applied your fix, but commented the part which added the fixed_in_version since this is not part of the simple page (at least for 0.19.0a2).

This is a bug I introduced while fixing another one where the product version field was missing from the simple view page.
